HEALTH INSURANCE TAX FORM

Notice of Availability of Form 1095-C

In compliance with the Affordable Care Act (ACA), we are required to provide Form 1095-C, which outlines the health insurance coverage we offered to our employees during the previous calendar year.   

Due to the Paperwork Burden Reduction Act enacted on December 23, 2024, employers are no longer required to automatically distribute Form 1095-C each year.  We are now only required to furnish Form 1095-C within 30-days from when requested.
